With the 2024 budget law, the contribution for voluntary registration with the National Health Service went up: for most people who must pay, the minimum is now €2,000 a year. It is one of the costs that most worries families wanting to bring elderly parents to Italy.

Who does NOT pay

Workers, people with permits for family, job-seeking, asylum or protection, those waiting for these permits to be renewed, unaccompanied minors and dependants of someone with compulsory registration: for them registration is compulsory and free.

Who pays

  • Students: €700 a year.
  • Au pairs: €1,200.
  • Other permit holders without compulsory registration (for example elective residence or religious reasons): at least €2,000, calculated as a percentage of income.

Registration runs for the calendar year, 1 January to 31 December, cannot be split and must be renewed every year.

Parents over 65

Parents over 65 who arrive through family reunification must have private insurance or register voluntarily. Some Regions apply different amounts pending a national decree: check with your ASL before applying for reunification, because the cost must be budgeted for.
